Patient Outstanding Balance & Payment Plan Guidelines

Payment will be requested at the time of service for all services that are non- covered or determined to be the patient's responsibility, including co-payments and deductibles from care you received at a rehabilitation facility or hospital. Payment may be made by cash, check, Master Card, or Visa.

We realize that deductibles are very high, and we will gladly set up payment plans. If you have a question regarding insurance, billing, or our fees, please call the office.

    Payment Plan Policy:

  • Patients with a large balance will be offered a payment plan.
  • Minimum monthly payments will be determined based on balance size, with 10% of the total balance as the starting point.
  • Failure to make a payment plan or comply with a payment plan will result in dismissal from the practice.

    Communication Expectations:

  • Patients should receive a monthly statement for any outstanding balance.
  • Our office will attempt to contact you before your appointment to discuss the outstanding balance.
  • Patients will be expected to pay the balance in full or set up a payment plan before their appointment.
  • Contact our billing department if you have any questions.

    Appointment Restrictions:

  • Patients with a balance over $250 and no active payment plan must set up a payment plan and make a minimum payment before scheduling non-urgent visits.
  • Urgent/emergency care will not be denied, but payment arrangements must still be made.

Insurance Claims/Billing

Medcorps Asthma and Pulmonary Specialist participates with most major insurance carriers. As a courtesy to our patients, we will file insurance claims for those insurances with which we participate. Please remember, any amount not covered by insurance is ultimately the patient's responsibility.

Prior Authorizations

Our office will complete any prior authorization paperwork required by your insurance company. Authorizations can take up to 14 days after all information is submitted. Specialty medications can take longer. We do not recommend scheduling your test without the authorization. Starting the prior authorization yourself or having another doctors office start the prior authorization will significantly slow the process.
